【業務内容】
①在宅訪問リハビリ
・利用者様のご自宅を訪問し、身体機能の維持・向上や日常生活動作（ADL）の改善を目的としたリハビリを実施します。
・医師や看護師と連携し、利用者様一人ひとりに合わせたプランを作成します。
②介護施設での訪問リハビリ
・介護施設に入居されている利用者様に対し、関節可動域訓練、歩行訓練、筋力強化などのリハビリを行います。
・施設スタッフとも協力し、より安全で快適な生活環境づくりをサポートします。
③多職種連携
・医師、看護師、ケアマネジャー、介護職員などと連携し、利用者様に最適なリハビリサービスを提供します。
④記録業務
・リハビリ計画書・報告書の作成や、利用者様の状態を記録・共有します。
