病院内にある地域医療連携室における相談・調整等の業務全般に携わっていただきます。
患者様やそのご家族と、病院や医師との間に入り、各種ご相談に対応する業務です。
相談内容の例：介護保険や福祉制度の説明、退院後の在宅医療に関する相談、施設入所の相談など多岐にわたります。
地域の関係各所との具体的な調整業務の実施
その他、透析患者様の送迎などのお手伝いを交替で行います。
透析業務全般を担当していただきます。
血液浄化業務、その他付随する業務の実施
